You may see and copy in person, or if you prefer, we will copy and send to you, the personal information about you in our files (except we will not share with you any privileged information, such as information collected in connection with a lawsuit or claim, and, should the law require, we may request professional approval before sharing certain medical information with you). We may make arrangements with an insurance support organization or a consumer-reporting agency to copy and disclose personal information to you on your behalf . If you advise us that any of the personal information we have about you in our records is incorrect, we'll reinvestigate and if we agree there was an error, we will correct it. If we don't agree, you're still entitled to file a statement with us, disputing the information. We'll send the correction or statement to anyone who received or will receive the original information. In order for us to safeguard your information, your request to see and copy such information must be in writing and include your name, address, policy number, daytime phone number, and a copy of personal indentification such as your driver's license.
